Woods, recovering from a fourth major back operation, passed two breathalyser tests but failed multiple field sobriety tests.
He later issued a statement citing an unexpected reaction to prescribed medications.
The 14-time major champion told officers at a testing facility that he had taken several prescriptions, and the arrest affidavit listed four medications, but not Xanax.
Portions of the incident investigation report were originally sensored for legal or security reasons by the police department.
But they have since reissued an unredacted version following a public records request by The Golf Channel.
Among the previously redacted portions was a note from Officer Christopher Fandry, who interrogated Woods on the scene.
I asked Woods if he had taken any medication to which he stated Xanax, Fandry wrote.
Fandry reported that Woods told him once more during the field sobriety test that he had taken Xanax.
After arriving at the testing facility, Woods told officers that he was on four medications but did not mention Xanax.
Since the incident 18-time major winner Jack Nicklaus, amongst others, offered to help Woods "in any way I can".
